Medical expenses claim

Hospital daily benefit claim

-Claim form
-Copy of receipts/ summary of medical bills that are certified as true and correct copy by hospital officials
-Medical certificate/medical report
-Copy of insurance card
-Certified copy of national ID card of the insured
-Copy of the first page of the passbook of the insured’s bank account (savings account/current account)

Loss of organ/disability insurance benefit claim

-Claim form
-Medical certificate/medical report
-Copy of insurance card
-Certified copy of police report concerning the case
-Copy of a complete medical record from the beginning
-Certified certificate of disability
-Photos evidencing loss of organs ** Photos must show the insured’s face and the organ loss**
-Certified copy of national ID card of the insured
-Copy of the first page of the passbook of the insured’s bank account (savings account/current account)

Accidental death benefit claim

-Claim form
-Copy of the house registration of the insured with the “deceased” mark
-Certified copy of medical certificate of death
-Certified copy of death certificate
-Certified copy of post-mortem examination report
-Institute of Forensic Medicine’s report / blood alcohol test report
-Certified copy of police report concerning the case
-Marriage registration/divorce registration
-Certified copy of national ID card and house registration of the beneficiary

Critical illness benefit claim

-Complete medical records from all hospitals that the insured has received treatment from
-Results of tissue biopsy/ CT scan/ MRI and blood tests
-Copy of national ID card

Claim for compensation for loss of or damage to baggage or personal effects therein

-Overseas travel insurance claim form
-Copy of boarding pass
-Copy of e-ticket
-Receipt of the lost baggage and personal effects therein or receipt of the baggage repair service
-Copy of police report listing the lost personal effects
-Letter of confirmation from the airline/hotel
-Proof of compensation for the loss or damage inflicted and the list of loss/damage

Claim for compensation for baggage delay

-Overseas travel insurance claim form
-Copy of boarding pass
-Copy of e-ticket
-Documents confirming the date and time of baggage pickup from the airport
-Letter of confirmation from the airline/hotel

Claim for compensation for trip cancellation and trip curtailment

-Overseas travel insurance claim form
-Copy of e-ticket
-Medical certificate in case of sickness/accident (the insured/offspring/father/mother (the insured/spouse))
-Copy of death certificate (the insured/offspring/father/mother (the insured/spouse))
-Proof of compensation from the airline/tour operator
-For cancellation by the airline

-Letter of confirmation from the airline/hotel

Third-party liability claim

-Overseas travel insurance claim form
-In case of accidental injury inflicted on third party

- -Confirmation letter from the injured
- -Medical report issued for the injured
- -Receipts of medical expenses
-In case of accidental damage to third party’s properties
- -Overseas travel insurance claim form
- -Confirmation letter from the victim that details the damages to his/her properties
- -Receipt of the repair service or, if the damaged properties are to be replaced, receipt of the damaged properties

Claim for compensation for flight delay

Claim for compensation for hijacking

-Overseas travel insurance claim form

Claim procedure

Treatment at a contracted hospital

-For treatment at a contracted hospital, please show your insurance card and ID card to the reception counter to check and verify your entitlement.
-Fill in the form provided by the hospital before seeing a doctor and receiving treatment.
-Check the expenses and sign the invoice.
-The hospital will collect the surplus and uncovered expenses (if any).

For payment made in advance or treatment at a non-contracted hospital

-Please request a medical certificate which must contain information about the disease/accident and treatment provide.
-Submit the documents to the following address:

Claim consideration period

The assessment takes 15 days after the Company has received all required documents.

Remark: If there are reasonable doubts that the insurance claims for compensation from the Company deviate from the agreed terms and conditions of applicable insurance policy, the process may be longer than the period specified above where necessary but will not exceed 90 days from the date all required documents have been received by the Company.
