The documents supporting the claims for compensation in the case of cancer and critical illness. In the case of a claim for cancer insurance benefits

• -Results of tissue biopsy / CT scan results / MRI results / Various blood test results

In the case of a claim for critical illness insurance benefits

For a claim for compensation for Acute Heart Attack (Myocardial Infarction) due to ischemia,
Medical documents are required, including a diagnosis from a doctor or a specialist in cardiology stating that the condition is Acute Heart Attack due to ischemia. Additionally, the claim should include the results of an electrocardiogram (ECG) and laboratory tests for Troponin (Cardiac Troponin) or Creatine Kinase-MB (CKMB) enzymes.

For a claim for compensation for a Major Stroke (Cerebral Hemorrhage or Occlusion)
Medical documents are required, including a diagnosis from a doctor or a specialist in neurology confirming that the condition is a Major Stroke (Cerebral Hemorrhage or Occlusion). This must be supported by the detection of new changes in the CT scan or MRI images.

For a claim for compensation for Chronic Kidney Failure,
Medical documents are required, including a diagnosis from a doctor or a specialist in nephrology confirming that the condition is Chronic Kidney Failure. The documents supporting the claims for compensation in the case of accidental death, disability, and loss of organ.
Accidental death claim

• -A copy of the ID card and a copy of the household registration of the insured, stamped with the word "Deceased."

Loss of organ/disability insurance claim

• -Photographs taken before and after the accident in cases of loss of limbs, loss of sight, or total permanent disability, showing the insured's face and the lost or impaired body part clearly.

Treatment at a contracted hospital

• -For treatment at a contracted hospital, please show your insurance card and ID card to the reception counter to check and verify your entitlement.
• -Fill in the form provided by the hospital before seeing a doctor and receiving treatment.
• -Check the expenses and sign the invoice.
• -The hospital will collect the surplus and uncovered expenses (if any).

For payment made in advance or treatment at a non-contracted hospital

• -Please request a medical certificate which must contain information about the disease/accident and treatment provide.
• -Submit the documents to the following address:

The assessment takes 15 days after the Company has received all required documents.

Remark: If there are reasonable doubts that the insurance claims for compensation from the Company deviate from the agreed terms and conditions of applicable insurance policy, the process may be longer than the period specified above where necessary but will not exceed 90 days from the date all required documents have been received by the Company.
